To begin the process, please complete our Voluntary Statement of Learning Needs<\/a><\/strong> form.<\/p>\n

Once completed, we recommend submitting it securely online<\/a>. However, you may also submit the form by email, fax, mail, or in person.<\/p>\n

The information you provide on this form helps us better understand your needs and ensures important details aren\u2019t overlooked during our conversation.<\/i><\/p>\n

Step 2:\u00a0 Submit documentation related to your disability<\/h2>\n

Please provide your most recent disability documentation before your appointment, if possible. This allows our staff time to review your materials and prepare for your meeting.<\/p>\n
