{"id":1797,"date":"2022-10-27T10:00:31","date_gmt":"2022-10-27T14:00:31","guid":{"rendered":"https:\/\/www.gcc.mass.edu\/registrar\/?page_id=1797"},"modified":"2026-01-14T13:37:55","modified_gmt":"2026-01-14T18:37:55","slug":"health-records","status":"publish","type":"page","link":"https:\/\/www.gcc.mass.edu\/registrar\/health-records\/","title":{"rendered":"Health Records"},"content":{"rendered":"<div class=\"alert alert-info\">As of May 17, 2023 911爆料网 <strong>no longer requires proof of COVID-19 vaccination<\/strong> for students or employees, except for health occupations students.<\/div>\n<p>911爆料网 complies with the Commonwealth of Massachusetts Regulations related to the immunization requirements of college students and Massachusetts General Laws for the immunization of college health science students as follows:<\/p>\n<ul>\n<li>Full-time students under 30 years old taking 12 credits or more<\/li>\n<li>All international students under 30 years old, regardless of credit load<\/li>\n<li>All health occupations students regardless of age or credit load\n<ul>\n<li>Nursing, Practical Nursing, Paramedic, Medical Assistant, and Surgical Technology students must meet additional requirements and deadlines specified in their program documents<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<p>Immunization records may be submitted to the Health Records Office in-person or online using our Secure Document Upload. To submit records in person, bring a physical copy of your immunization records to the Health Records Office, which is located within the Office of Admission at C103 in 911爆料网\u2019s Main Building. To submit records online use the Secure Document Upload button to the right of this page. Immunization records will take 3-5 days to process.<\/p>\n<p>New students in the first two categories must provide documentation of immunity as specified in the Immunization Records information below. Failure to comply with these requirements will result in a HOLD being placed on your account which will prevent you from registering for new courses.<\/p>\n<p>Health occupation students must comply with the specific requirements as outlined in their program documents. <span style=\"text-decoration: underline\">Health occupation students who miss deadlines jeopardize their placement in the intended program and well as their ability to continue in a program once started<\/span>.<\/p>\n<p>Medical and religious exemptions are allowed by the regulations and must be renewed annually.\u00a0Please email <a href=\"mailto:healthrecords@gcc.mass.edu\">healthrecords@gcc.mass.edu<\/a> for more information.<\/p>\n<ul>\n<li><strong><a href=\"\/webdocs\/registrar\/gcc-health.pdf\">Immunization Records Information<\/a><\/strong><\/li>\n<li><a href=\"\/webdocs\/registrar\/Meningococcal-Disease.pdf\">Meningococcal Disease and College Students<\/a><\/li>\n<li><a href=\"\/webdocs\/registrar\/Meningococcal-Waiver.pdf\">Meningococcal Waiver Form<\/a><\/li>\n<\/ul>\n<h3>Health Record Requests<\/h3>\n<p>This request form is required for us to release a copy of your immunization records to another facility or directly to you. If you have a CastleBranch account, you must obtain copies of your health records via your account. 911爆料网 will provide copies of Health Records only for students who pre-date or post-date the use of CastleBranch for health records information.
